  Clients who cannot attend clinic person are encouraged to contact me to arrange a telephone nutritional consultation - you will need to complete the Nutrition Questionnaire form and return it to me. Please call the Dolphin House Clinic on (+44) 01273 324790 to request a copy, or

download the nutritional assessment questionnaire to print out and complete at home.

  The first nutrition consultation lasts up to 1½ hours and a detailed case history is taken which includes diet, lifestyle and current state of health. Follow-up appointments in which progress is reviewed and appropriate changes are made last about ½ hour.

  Fees: I charge only for my time and professional expertise. Please call the Dolphin House Clinic for my current rates, and whether any concessions may be available according to your circumstances. I do not sell vitamin or mineral supplements. If supplementation is considered necessary I advise my clients on where they may purchase direct.

  Each person is individual, but it is usual for a nutrition programme to be supervised over one year with an average of 4 visits. Specific nutritional tests may be recommended during consultation, such as a hair mineral analysis or allergy tests. These tests are optional.

  A course of dietary supplements may also be recommended for a specific period of time in addition to any dietary modifications. Individual dietary and financial circumstances are always taken into consideration when devising a nutritional programme.
